Requirements
What a certified translation must include
- A complete, word for word English translation of the entire document, including seals, stamps, signatures, and marginal notes.
- A signed Certificate of Translation Accuracy stating that the translation is complete and accurate and that the translator is competent to translate.
- The certificate carries the printed name and signature of the qualified professional translator, the language pair, the date, and the order reference.
- A QR code on the certificate opens a public verification record for the order.

How Dutch-language documents work
In the Netherlands, civil status records (akten van de burgerlijke stand) are drawn up and held by the municipality (gemeente) where the event occurred. Copies and extracts of birth, adoption, marriage, divorce, death, and registered partnership certificates are requested from that municipality; adoption certificates are only available via the municipality of The Hague.
Dutch municipalities issue two forms of a civil status record: an afschrift (a full certified copy of the original act, including marginal annotations) and an uittreksel (an extract with the core personal data only). The two are distinct documents and translators name them differently.
Dutch municipalities can issue an international (multilingual) extract of a birth, marriage, or death record with field captions in ten languages: Dutch, English, French, German, Spanish, Greek, Italian, Portuguese, Turkish, and Serbo-Croatian.
The ten-language international extract format used by the Netherlands and Belgium is the multilingual extract under ICCS (CIEC) Convention No. 16, signed at Vienna on 8 September 1976. Even with a multilingual extract, US agencies generally still require a full certified English translation of any content that appears only in Dutch, such as annotations and marginal notes.
